Our nurses are trained to identify potential wounds, educate you and your caregivers on wound care management, and prevent wounds and wound care complications.  We work closely with you and your Physician to create the best treatment plan for you. We provide cutting edge technology and advanced wound care products. We also have a team of extremely talented Certified Wound Care Consultants.  Your nurse will coordinate a treatment plan with your physician on wound vac management if appropriate.  We also coordinate with wound care clinics as a member of your health team.

Advanced Dressings & Supplies:

Where others cut costs, Hometown invests in success. We use the most advanced, highest quality wound care supplies available on the market. Supplies such as calcium alginates, hydro fiber, foam dressings, hydrocolloids, and wound gels. These dressings provide antimicrobial activity, improved fit over difficult anatomical areas, faster healing, and improved patient comfort.

Vacuum Assisted Closure:

While some agencies shy away from the advanced technologies to assist with wound healing, Hometown works harder to give your patients the best wound care available. Vacuum assisted closure therapy is a complex wound management system used in homecare settings that promotes angiogenesis while preparing the wound bed for closure.

Electrical Stimulation:

HVPC E-Stim provides Hometown another edge in the treatment of difficult to heal wounds. This wound care modality is recommended by the Agency for Health Care Policy and Research and by Medicare for certain types of wounds. Hometown is one of the few, if not the only, home health to make it available in the home. E-Stim has multiple effects on a wound site including increasing collagen tensile strength and increasing protein synthesis. This ultimately results in new tissue growth or granulation.

Ultrasound Wound Care:

Hometown staff can use specific ultrasound to accelerate wound healing. Our ultrasound can cause degranulation of mast cells, releasing histamine, and attracting neutrophils and monocytes. Ultrasound can also cause targeted heating that will improve circulation resulting in the progress of wound healing.

Anodyne Therapy:

We use this to stimulate wound healing in chronic, non-healing wounds. Anodyne delivers near infrared monochromatic light directly to the wound site. This energy inspires red blood cells to release nitric oxide, a mediator in several healing processes.

Hometown Treats Both the Wound and the Patient

Holistic care is our focus when we treat a patient with a wound. We help the patient and teach the caregiver as a whole. From the Hometown Home Health perspective, truly successfulwound care includes the avoidance of emergent care and infections during the   home health episode and the avoidance of re-injury in the future. Hometown invests heavily in supplies and equipment to prevent infections and more aggressively manage existing infections. Hometown utilizes the expertise of Wound Certified Nurses to assess,develop, and implement a wound care protocol specific to the patient and his or her wounds. OT, PT, MSWand dietitians are available to modify the home environment and train patients and caregivers to avoid similar problems in the future.   Hometown evaluates rehabilitation potential for wound care cases; even modest improvements in physical ability can promote wound healing and prevent future problems.
