Staying in a hospital can leave you feeling weak and uncomfortable. The Hospital Admission Prevention Program provides a proactive approach to keeping you from going to, or returning to the hospital. Our skilled team will coordinate with your physician to establish a patient-specific plan of care. This plan will not only focus on the primary reason for your care, but for improved health and improved patient outcomes. Our nursing team is always available, which allows us to prevent unnecessary hospitalizations.
Hometown understands that knowledge improves care, thereby reducing hospitalizations. Hometown is recognized by the State of Michigan as an authorized provider of Continuing Education at AFC/ALF’s and as a teaching organization. We are certified to provide continuing education hours to various professionals. Hometown can provides teaching and in-services to caregivers as well.
Hometown encourages facilities to organize in-services throughout the whole year and recommends management and evaluation programs such as Fall Prevention, Skin Integrity, CHF, Heart Conditions, Medication Administration & Behavioral Problems. Education is also provided regarding the full scope of Hometown’s services.
