Hometown Home health and Hospice specializes in providing an individual In-Home Behavioral Management program that assists patients and caregivers to restore and promote mental, physical, and emotional wellness. This program provides trained nurses who observe and assess physical and mental needs, responses to medication, coping strategies, behavior changes, and supervise and coordinate other needed services such as Occupational and Physical Therapy, Speech Therapy, Home health Aides and Medical Social Workers.

Hometown’s Nursing team can provide a thorough psychiatric assessment by utilizing our advanced knowledge, screening tools, and experience. This allows our Nursing team to establish patient / caregiver specific education and interventions.

Our Behavioral Management Program Requirements:

Our Behavioral Management program consists of highly trained and experienced Registered Nurses who provide thorough psychological assessments, utilizing their advanced knowledge, screening tools, and experience.  Our behavioral management team assists patients with a new psychiatric diagnosis, as well as patients who have had chronic mental health challenges. We provide psychological support and education focusing on coping mechanisms and medication compliance. We can assist with coordinating your mental health care team and monitor therapeutic effectiveness of physician ordered medications. Our team may also include a Social Worker, if appropriate. We promote holistic care, which includes mental and physical health.

Our goal is to promote mental, physical, and emotional wellness. Interventions will be created that will assist with hospitalization prevention.

Medicare typically covers 100% without and deductables or co-pays.

We participate with most insurance policies.
