Q: What is Homecare?

A: Many of the services you receive in a hospital or nursing home can be had in the comfort of your own home. Hometown Home Health can send nurses, therapists, home health aides, medical social workers, and other health care professionals to your home. In the more comfortable, less costly environment of your own home, Hometown’s caring professionals help with wound care, rehabilitation, review of medications and special diets, care after a surgery or medical procedure, and much, much more.

Q:   How do I know if I qualify for Homecare Services?

A:  Medicare regulations for home health care state that a patient must need the skilled services of a nurse or therapist. A patient must also qualify as homebound (i.e., leaving the home requires taxing effort, and trips outside the home are infrequent).  For all insurance providers, a physician must authorize the home health care by writing a prescription.

Q: What types of services does your agency provide?

A: Hometown provides Skilled Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Social Work, Home Health Aides, and Dietary Consultations.  For more information on these specific services, please contact us at 888-852-3310.

Q: What is a Face-to-Face Encounter?

A: For Medicare patients, you are required to have seen your physician (face-to-face) within 90-days prior to your Home Health Care Admission or within 30-days after your Home Health Care Admission date for coverage of homecare services.  The reason for the visit with your physician should relate to the reason why home care services are being ordered.  At this time, only patients with Medicare are required to have the Face-to-Face Encounter form completed by their physician.
